FAMILY PARTNER I - YOUTH RESOURCE CENTER (FPI-YRC)

JOB DESCRIPTION

Division: Families F.I.R.S.T.   Date: September 15, 2009  FLSA Status Non-Exempt

 

PURPOSE

 

Performs a variety of duties and responsibilities to maintain the efficient and safe operation of the facility, its’ youth and their related activities in accordance with the program standards.

 

ACCOUNTABILITIES

 

Demonstration of sound judgment, positive working relationships with the youth, staff and other personnel.

Assures all youth activities are done according to protocol and procedures.

Completes all required paperwork in a timely and accurate fashion.

Complies with all facility policies and procedures in line with contractual and licensing requirements.

Adhere to Person/Family Centered practices at all times.

Maintain fidelity with the chosen EBP/PP/Best practice Models.

Upholds the mission, philosophy and goal(s) of Visinet and the Families F.I.R.S.T. Model at all times.

Works harmoniously with staff in the center and others.

Assures facility security.

Follows appropriate chain of command and protocol according to situations.

Utilization of both preventive and interventions strategies with regard to specific situations.    

 

ESSENTIAL FUNCTIONS OF THE POSITION AND PERFORMANCE STANDARDS

 

Assures youth who are residing in the facility comply with policies and procedures to assure their safety as well as others in the facility.

Follows the YRC Program Model and consistently applies the Model at all times while working with youth and families receiving services in the YRC.

Documents all significant activities, appointments and contacts including reports of individual, group or family discussions or visits in the youth’s file.

Oversees and supervises all youth activities to include meal preparation, group dining, chore routines, educational tutoring, sleeping, recreational and community service activities.

Completes daily and monthly reports and logs to reflect thorough documentation on all youth including reporting any significant problems, or activities which require the attention of and/or investigation by the Facility Manager.

Assists in managing crisis situation for youth in the center according to established guidelines and notifies appropriate persons of the crisis to include non facility personnel.  Follows Emergency Protocol as outlined in EM.

Assists in obtaining or scheduling age appropriate recreational or community service activities for youth while at the facility.

Participates in meetings, case reviews, training and related sessions that are deemed appropriate for the program success.

Participates in mandatory Family Team Meeting within 72 hours of admission.

Assists in the transportation of youth to various scheduled appointments or activities.

Assists in assuring security for the facility by participating in regular drills and daily checks.

During evening or overnight shifts is alert, responsive, and follows all YRC program policies and procedures.

Performs other duties as may be assigned.

Quality of services and reports.

Rapport with family and referral sources.

Timely attendance at scheduled sessions, meetings.

Assurance of required trainings.     

 

 

 

 

RELATIONSHIPS

 

Supervised by:  Families F.I.R.S.T. Manager - YRC

Supervises: N/A

Internal: Works in harmony with all team members, supervisors, managers and other related personnel.

External: Frequent contact with outside professionals as need arises.  Families and other providers involved with youth.

 

JOB SPECIFICATIONS AND WORK REQUIREMENTS

 

Education

Required: Associates degree in human services or related field.

Desired: Minimum Associates degree in child development, behavioral counseling or related field.

 

Experience

Required: Must be at least 21 years of age with a minimum of 3 years experience working with children and families.

Desired: Prior group home or institutional experience preferred.

 

Licenses, Certifications, and/or Registrations: Must have valid driver’s licenses with good driving record. Clear Criminal History Background and Adult/Child Abuse checks, as well as all program specific certifications, Model specifics, NCI, First Aid/CPR, Care Restraint, Defensive Driving (Training offered by Visinet, Inc. and its subcontractors).

 

Equipment/Tools/Work Aids: Knowledge of Microsoft word and basic computer use helpful.

 

Specialized Knowledge and Skills: Spanish, Sign Language, etc. beneficial.

 

Working Conditions:

Works in a controlled environment.

Limited exposure to weather related elements depending on seasons.

From time to time, will be subject to elements of outside, i.e., potentially aggressive youth.
