Definition?
Intensive Family Preservation (IFP) provides a more intensive level of service to families who may need additional support beyond typical family support or outpatient services. IFP may be accessed as a transitional service for families with children returning home from hospitalization or residential placement; or, it may be utilized to prevent out-of-home placements.
Service goals of Visinet, Inc.?s IFP Program include: 1) to enhance the parents? ability to create a safe, stable, and nurturing home environment that promotes healthy child development and permanency; 2) to prevent out-of home placement of children when safety can be established for all family members; and 3) to provide, refer to, and coordinate services needed to achieve or maintain family safety, stability, independence, and unity.
Visinet, Inc.?s IFP team, which is made up of a therapist and skill builder(s), make multiple contacts with the client/family on a weekly basis providing assessment, treatment, support, education, resource enhancement and progress/discharge services.
The IFP program is outcome-based and time-limited to fifteen (15) weeks. As part of this service, Visinet, Inc. also provides twenty-four (24) hour, seven (7) day a week crisis intervention services.
How do I access services?
This service requires a referral and authorization from Nebraska Health and Human Services (NHHS)/Integrated Care Coordination Unit (ICCU), Region case managers and probation/Office of Juvenile Services (OJS) for Visinet, Inc. to implement services.
The entire service authorization is fifteen (15) weeks, but may be sought in two phases?seven (7) weeks and eight (8) weeks. The authorization must be for two (2) occurrences for each phase for a total of four (4) occurrences for the fifteen (15) week cycle.
What are the criteria?
Families who qualify need to be involved with NHHS/ICCU/the Region/probation/OJS. IFP may be used as an assessment tool (the first seven (7) weeks; or, as a transitional service for families with children returning home, and/or to prevent out-of-home placements (the entire fifteen (15) weeks).
Payment?
Funding is made available through NHHS, ICCU, the Region, and probation/OJS.
