JOB DESCRIPTION
Division: Families F.I.R.S.T. Date: September 15, 2009 FLSA Status: Non-Exempt
Performs all the duties as described in Family Partner, Level I as well as a variety of duties and tasks to coordinate, and administer services according to case plan to prevent abuse/neglect of children, strengthen families and/or assist in providing permanency for children in care as outlined in program model.
1. Assures safety of children through continual use of visual and audible skills.
2. Maintains fidelity with the chosen EBP/PP models.
3. Adheres to Person/Family Centered practice at all times.
4. Performs task by demonstrating professionalism at all times.
5. Demonstrates respect and shows dignity to all children/youth and families
6. Assures confidentiality of information.
7. Assures case plans goals and objectives are implemented within service provision.
8. Complies with all corporate and contract policies and procedures.
9. Completion of all documentation as dictated by program within identified time frames.
10. Maintains current status for all training related to program and position.
11. Decision making skills within scope of job performance.
ESSENTIAL FUNCTIONS OF THE POSITION AND PERFORMANCE STANDARDS
1. Prepares, plans and manages cases to assure case plan is being carried out for children/youth and families by reviewing case information, making appropriate contacts, and establishing rapport with the children/youth and families.
2. Monitors child/youth and parent/caregiver interactions to ensure the safety of all family members and the community.
3. Assists in locating suitable alternative placement to meet a specific child’s needs and to meet policy and program specific guidelines.
4. Documents events and occurrences during sessions relating to inter-actions between parents, child(ren), or child/guardian.
5. Transports children safely using approved and appropriate car restraints.
6. Provides direct service care within the Alternative Family Mentor Care homes.
7. Assists in locating suitable alternative placement to meet a specific child’s needs and to meet policy and program specific guidelines.
8. Develops, recommends, implements, monitors and evaluates family case plan and makes changes, where applicable, in conjunction with service coordinator and according to family team recommendations.
9. Maintains on-going contact with Families F.I.R.S.T. Manager, CFS, Service Coordinator regarding each case assigned as deemed appropriate or as requested.
10. Attends program specific training, and/or meetings as specified by supervisor.
11. Assists with coverage for other staff in the event of an emergency or other specified need.
12. Completes all reports in a timely, accurate and efficient manner according to program standards to include drafted court reports.
13. Attends court when requested to by Families F.I.R.S.T Manager and DHHS/CFS.
14. Attends family team meetings or team staffing when requested to do so.
15. Elevates session and/or safety concerns immediately to supervisor or program director.
16. Assures in-home safety of potential crisis situations in the family unit at varied hours throughout the day or night.
17. Performs other tasks as may be assigned.

ü External: Continual contact with families, CFSS, Service Coordinators, and other stakeholders involved with the child/youth and family.
JOB SPECIFICATIONS AND WORK REQUIREMENTS
1. Education
a. Required: Minimum of Bachelor degree in human services or related field.
b. Desired: Minimum of Bachelor degree in Human Services or Childcare.
2. Experience
a. Required: Must be at least 21 years of age with a minimum of 3 years experience working with children and families.
b. Desired: Prefer a minimum of five years experience in a field directly related to children and/or families.
3. Licenses, Certifications, and/or Registrations: Clear Criminal History Background and Adult/Child Abuse checks, as well as all program specific certifications, NCI, First Aid/CPR, Car Restraint, Defensive Driving, clearance from all background checks and drug screens (Trainings offered by Visinet, Inc.)
4. Equipment/Tools/Work Aids: Knowledge of Microsoft word and basic computer use helpful.
5. Specialized Knowledge and Skills: Spanish, Sign Language, etc. beneficial.
6. Working Conditions:
a. Varied climates both indoors and outside in homes, neutral locations or related areas.
b. Varied working hours – typically late afternoon and early evenings and at least one weekend day.
c. Various other components such as pets, housekeeping skills, etc.
d. Significant time spent driving between and/or to sessions.
