Family Partner - Content Section 1 (ID:160)

FAMILY PARTNER:  Bachelor?s degree in a human service field and six months experience working with at risk children and families.


  • Contact all parties within twenty four (24) hours after receiving a new referral or within timeframe dictated by program/referral.
  • Provide a variety of services including: Intensive Family Preservation, In-Home Safety, Family Support.
  • Responsible for monitoring the Safety Plan and providing ongoing measurement of Protective Capacity.
  • Serve as the school liason.
  • Assist the client/family in obtaining the goals identified by the referral source and illustrated on the case plans (referrals). 
  • Develop in conjunction with your supervisor creative interventions while addressing established goals with the ultimate goal of reunification and stabilization.  
  • Responsible for the successful outcomes of children and families as well as accept the penalization of negative outcomes.
  • Remain within the chosen EBP/PP models.
  • Remain within the guidelines of the referral at all times, as well as, within the required authorization period and number of hours allocated to the client/family. 
  • Implement and utilize resources during all sessions which relate to the identified goals on the client/family case plan and as dictated by the service referred. 
  • Maintain a strengths-based approach while working with client(s) and families to assist in the retention of dignity and the development of positive intrinsic qualities. 
  • Adhere to the emergency protocol (found in the Employee Manual) in the event that a client or individual associated with the client discloses abuse/neglect information. 
  • Maintain appropriate boundaries and confidentiality with client(s) and families, as well as, with all team members involved on a particular case. 
  • Adhere to the strict confidentiality policies indicated in the Employee Manual, as well as, all of the other policies and procedures.
  • Adhere to family centered practice.
  • Consistently review Continuous Quality Improvement (CQI) plan and make sure that all it is being carried out within the program on a daily basis. 
  • Ensure that Health Insurance Portability and Accountability Act (HIPAA) standards are maintained throughout the program. 

Starting pay is $13.46/28K.
